At the very least, astrotwins share most sign and degree placements of most planets, and most aspects. At most, astrotwins could share the same sign and degree placements of all planets, the same aspects, and the same sign and degree placements of the ascendant and the other house cusps.
The most exact astrological twin a person could have would be someone who was born not only on the same month, day and year, but who was born in almost the same location (such as the next hospital bed over), and at the same time. This would give both people virtually the same chart.
Such hypothetical astrological twins, born to different mothers at almost the same location and the same moment, would have charts that are even closer to being exactly the same than the charts of most biological twins. Most biological twins' charts would not be quite that exactly the same, because there is generally a bit of time elapsed between the births of two twins from the same mother.
It is possible for a person to have an astrological twin who was born in a relatively distant location, at a different time on the same day, who nonetheless has a very similar chart. (That is, a chart with the same sign and degree placements of the ascendant, and similar house cusps, in addition to most of the same sign and degree placements of the planets).
For example - all of the following chart data, from January 1, 1980 at various locations and times, produces charts with 19° Aries rising and most of the same signs on the cusps of the other houses, if not the same degrees exactly. (Though the signs and degrees of the house cusps depend, in part, upon the house system you use). Possibly the most significant difference in these charts is the position of the quickest-moving astrological body, the moon.
