Do you mean natally or by transit? I'm thinking you must been natally (born wiht the aspect in the birth chart).

Neptune in Leo conjunct the North Node in Leo in the 6th house might mean that there is potential for someone to work in healing in this life, maybe through an energy (warm or solar). Work could also be theatrical or artistic, as Leo and Neptune blended lead the fantasies or imaginings to become acted out or "displayed". So there may be a path of advancement through the North Node that is theatric or artistic in that way. Or it might mean that there is often confusion or distraction and lack of focus in the work, but that through the Node this person must find a higher path.

Another thing it could mean is that someone could authorize or administer assistance to the poor, helpless or dying, and that the potential for advancement of the soul is through that work. The 6th house, in addition to work and service to others, is about one's health. So this could mean a Neptunian disability around the heart or back (both Leo) that leads one to compassionate healing and a purpose in one's future.

There are a multitude of things that just this one placement could mean, but it you synthesize it with the rest of the chart it should become clearer. Check aspects from other planets to Neptune in and the North Node as well.
